Incoming Questionnaire

We thank you for your commitment to raising a PAWS foster puppy. Now is the time to prepare for the foster puppy’s return for formal training at PAWS national headquarters. We ask that you kindly complete this questionnaire based on the last three to four months the puppy has been in your home. Your answers to these questions will help the PAWS staff to know more about the foster puppy and prepare for the pup’s continued training for a career.
